FDA is requiring stronger warnings about an increased risk of lymphoma and other cancers in children and adolescents who are treated with drugs that block tumor necrosis factor. TNF blockers include Remicade (infliximab), Enbrel (etancercept), Humira (adalimumab), Cimzia (certolizumab pegol), and Simponi (golimumab). These drugs are used to treat juvenile rheumatoid arthritis, Crohn’s disease, and other inflammatory diseases.

The warnings, which include an updated boxed warning, are based on an FDA analysis of reports of cancer that occurred in children and adolescents treated with TNF-blockers. The cancers, some of which were fatal, occurred after an average of 30 months of treatment. About half of the cancers were lymphomas. The labeling will also include updated information on cases of leukemia and new onset psoriasis associated with TNF-blockers.

Healthcare professionals should monitor patients for development of malignancies during and after treatment with TNF blockers. They should also talk to patients and their families about this risk and tell them to report signs and symptoms of cancer such as unexplained weight loss or fatigue, easy bruising or bleeding, or swollen lymph nodes in the neck, underarms or groin.

Patients should also be monitored for signs or symptoms of new onset psoriasis or worsening psoriasis. The patient Medication Guide will be updated with this new information.

Your doctor has recommended that you have a lower GI endoscopy. But what does that actually mean?

A lower GI endoscopy is a diagnostic procedure used by your doctor to inspect the inside of your rectum and colon. While it’s considered a surgical procedure, endoscopy does not involve an incision.

Instead, your doctor will pass a flexible tube, called an endoscope through your anus and into your rectum and colon.

This tube has a tiny video camera mounted on its tip.

It also contains a small tool used for taking tissue samples.

Your doctor can use the endoscope to inspect the entire lower half of your digestive system.

In some cases, the shape of the colon makes it impossible to pass the endoscope as far into the body as the doctor would like.

Your doctor may decide to take a series of x-rays – or even to perform surgery – in order to inspect the hidden area.

Reasons for undergoing a lower GI endoscopy vary. You may have been suffering from one or more of a number symptoms – including blood in your stool, weight loss, chronic irregularity or other problems associated with the digestive system.

Some gastrointestinal symptoms can be warning signs of serious medical problems and you should take your doctor’s recommendation to have an endoscopy very seriously.

Luckily, the vast majority of medical problems diagnosed by endoscopy are treatable and you should look forward to improved health and comfort as a result of the information gathered during the procedure.

The doctor will then lubricate your rectum
and gently insert the endoscope, guiding it into your colon.
You may feel some pressure or tugging, but you shouldn’t feel pain.
To better examine abnormal-looking tissues,
your doctor may choose to take one or more biopsies. Patient Education
Small instruments sent through the interior of the endoscope are able to painlessly remove small samples of tissue with a small scissor like tool by simply snipping them free.
Finally, after a thorough exam, the endoscope is carefully removed.
Any tissue specimens removed during the procedure will be sent immediately to a lab for microscopic analysis. Your doctor will tell you when to expect results from those tests.

Lower GI endoscopy only rarely leads to complications.

The main risk is creating a tear – with bleeding as a result. In most cases, bleeding like this will stop on it’s own. In extremely rare cases, further surgery may be required. At home, you should be able to resume normal activities, as you feel able.

* Finally, you must also arrange for transportation to and from the place at which the procedure will be performed. Everyone reacts differently to even simple surgical procedures and there’s no way you can predict how you’ll be feeling after the procedure. So don’t forget, following surgery, you will not be allowed to drive.

The nurse and anesthesiologist will be closely monitoring your comfort and progress. Remember, don’t hesitate to ask any questions about any of the information that they’ve just provided.

Before you leave, you’ll be given discharge guidelines which may include diet, medication, work and other activity restrictions.

You’ll also make at least one follow-up appointment so that the doctor will be able to check the healing of the incision and/or to remove sutures.

The notion that ovarian cancer is a “silent killer” without early warning signs has changed.

The Gynecologic Cancer Foundation has announced a national consensus on symptoms for ovarian cancer. Symptoms that women should consider as warning signs include: bloating, pelvic or abdominal pain, difficulty eating or feeling full quickly, and frequent or urgent urination. If women have these symptoms daily over the course of several weeks they should consult with a gynecologist about them, particularly if the symptoms are new or unusual.
